Notice that the first line copies Aadhaar, the marriage line copies the certificate, and the new name appears three times spelt identically. Point 3, the "one and the same person" line, is what makes this affidavit useful years later for a school certificate that still carries your maiden name.

Always the full name, both times. Even for a surname-only change, point 2 carries your complete maiden name and your complete married name. "Changed my surname from Debnath to Chakraborty" is not accepted by the Gazette office and confuses every other one.

Part IV

Maiden name
The name a woman held before marriage. A marksheet or degree in a maiden name is normal and is explained by the marriage certificate rather than corrected.
Marriage certificate
The Registrar's certificate naming both parties and the date of marriage. It proves a surname taken on marriage and nothing beyond that.
Affidavit
A written statement you swear is true before a notary. For a name change it records your old name, your new name, your parent's or husband's name, your address and the reason.
Notary public
A person authorised by the Government to witness signatures and certify documents. Notarisation is done in person in Tripura and is the one step that cannot be completed online.
Proforma
The typed page that becomes your Gazette notice. It is filed in two identical copies, signed in your existing old name, and witnessed by two people.
Newspaper notice
