The Gazette office prints exactly what your proforma says. One wrong word and the Gazette of India carries the wrong name for the rest of your life. Three things to notice in the example. The old name is written the way it appears on the applicant's Aadhaar, spacing included. The address is written in full, down to the district and pin code, exactly as on the address proof. And the signature line carries the old name, because on the day of filing that is still the legal name.

A child's notice is not the adult form with the child's name typed into it. It is a different sheet, made by the parent.

Four things separate this from the adult sheet. The parent is the applicant throughout, so the first line carries the parent's name and address, not the child's. The child's age is written into the notice itself. The signature is the parent's, in the parent's own name, and the child signs nothing at all. And the affidavit that accompanies it carries one extra line the adult version does not have, stating the child's date of birth.
Public notice format when a name has been wrongly written
Not every Gazette notice is a change of name. When a record carries a name that was never yours, the wording is different, and so is what you are asking for.
A change of name says that you were one person and are now called something else. A wrongly written name says the opposite: nothing about you changed, a clerk wrote the wrong thing, and the record should be amended to what was always true. The Department accepts both, but the notices read differently, and using the change wording for a correction is a common reason a family ends up publishing twice.

Why this version saves a Tripura family money
Read the wording again and notice what it does. One notice names three people, states where the wrong entry sits, gives the correct names and asks that the record be amended. A household that has been recorded as Gidh in a school register when the family name is Bhagat does not need three separate publications, three sets of newspaper notices and three Government fees. It needs one notice, correctly worded, naming everybody affected.

Public notice when the name is wrong across several certificates
One notice, every certificate named, and the father's name corrected alongside your own.
This is the version that education cases actually need, and it is the one most typing shops get wrong. A student rarely has a single wrong certificate. The same error usually runs from Class 10 through Class 12 and into the degree and the postgraduate certificate, and it usually affects the father's name as well as the student's, because both were copied from the same admission form. Publishing four separate notices for four certificates is a waste of four Government fees. One notice, drafted properly, names each certificate in turn and corrects both people.

The rule that makes this notice work
Recite each certificate's wrong spelling exactly as that certificate prints it, punctuation included. A name may appear as S Nizamuddin on the Class 10 certificate, as Nizamuddin. S on the Class 12 one and as Nizamuddin S on the degree. Those are three different strings, and a board will only amend the one it can see quoted back at it. Do not tidy them, do not standardise the spacing, and do not merge them into a single description. Copy each from the certificate in front of you.

Newspaper advertisement format for a Tripura name change
Two notices, one in a Bengali daily and one in an English daily, worded exactly like your proforma.
The newspaper stage is where most money is wasted, because people either write the notice themselves in their own words or let the paper's counter shorten it to save column space. Both create the same problem: a published notice that does not say what the proforma says. The wording below is the same sentence you are filing, which is the point.

What the newspaper stage must get right

For a minor the notice is placed by the parent and follows the minor wording, and for a wrongly written name it follows the public notice wording above. We book both notices for our clients so that the text going to the paper is the text already approved for the proforma, which removes the commonest reason a Tripura file has to be republished. There is no rule that a particular newspaper must be used, and anybody insisting on one specific paper is selling you their arrangement rather than a requirement.
The affidavit format that goes with the proforma
The affidavit is sworn on non-judicial stamp paper before a notary in your district. It is short. What matters is that the old name, the new name, the parent's or husband's name and the address are word for word the same as the proforma.

For a marriage case, point 1 reads that you have changed your name from your maiden name to your married name after your marriage on the date shown. For a minor, the parent or guardian is the deponent and point 1 reads that they have changed the name of their minor child. The CD certificate format for the Gazette file
The page most people have never heard of, and the one that trips a file when it is missing.
The Department asks for the proforma text as a soft copy on a CD, in MS Word, so it can be set for printing without retyping. Because the printed page and the CD must match, you sign a short certificate taking responsibility for that match.

The CD itself carries the proforma text only: the "I, hitherto known as" matter, the certification line, and the old name typed where the signature sits. The two witness blocks are left off the soft copy. If the CD text differs from the printed proforma by a single character, the certificate you signed says the mismatch is your responsibility, and in practice the file is returned.

Filling the Gazette proforma from Tripura, step by step
Copy the old name from your ID
Open your Aadhaar or passport and type the old name exactly as printed, including spaces and initials. Do not "correct" it, even if you think it is wrong. That is the name the Gazette is changing from.
Write the new name once, and lock it
Type the new name with the spelling you will use for the rest of your life. Write it on a piece of paper too. Every other page copies from this line.
Fill the relationship and the address
Son of, daughter of or wife of, then the name as on your ID. Then your full address from your address proof, with district, Tripura and the pin code.
Print two copies and sign in the old name
Computer typed only. Sign both copies in the existing old name, in the space above the words "Signature in existing old name".
Two witnesses sign both copies
Any two adults. Each writes their signature, full name and address on both printed copies. Attach a self-attested copy of each witness's ID.
Make the CD and sign the certificate
Save the proforma text in MS Word without the witness blocks, with the old name typed in the signature place, on a CD. Sign the CD certificate.
Then the affidavit, the request letter, the newspaper pages, the IDs, the photographs and the BharatKosh receipt join it, as listed on the documents required page.
Spelling rules for Bengali and Kokborok names on the proforma
The proforma is typed in English, and English has no fixed rule for spelling a Bengali or Kokborok name. That is why the same family can hold documents reading Debbarma and Deb Barma, Reang and Riang, Chakma and Chakmaa, Bhattacharjee and Bhattacharya. On the proforma you need two decisions, and you need them in writing before anything is typed.
The old name
Not a decision at all. It is whatever your ID says, letter for letter, even if it is the spelling you dislike. The Gazette changes from that name, so that name must be recognisable to the Aadhaar centre and the bank.
The new name
One spelling, one spacing, one decision about initials and dots. Once printed in Part IV, that is the spelling every office in Tripura will match against. If you want "Debbarma" as one word, type it as one word on the proforma, the affidavit, both newspapers and the CD.

- A formal announcement printed in the official journal of the Government. For a name change it is the document Aadhaar, PAN, passport offices, banks and employers accept as proof that the change was made.
- Central Gazette
- The Gazette of India, published by the Department of Publication at Civil Lines, Delhi. It is accepted in every office in India, including every office in Tripura, and by foreign missions.
- Tripura Gazette

- Affidavit
- A written statement you swear is true before a notary. For a name change it records your old name, your new name, your parent's or husband's name, your address and the reason.
- Deponent
- The person who swears an affidavit. For a minor's name change the deponent is a parent, never the child.
- Verification clause
- The closing paragraph of an affidavit stating the place and date and confirming that the contents are true to the best of your knowledge and belief.
- Two witnesses
- The Gazette notice carries two witnesses, each giving a signature, a full name and a complete address. Initials or a partial address are a common reason a file is returned.
- Newspaper notice
- The same wording published in one Bengali daily and one English daily circulating in Tripura. The complete original page of each is kept, because a cutting is refused.
