Why a name change needs a newspaper notice
The notice exists so that anybody with an interest in your old name has a fair chance of seeing that you now use a different one. A creditor, an employer, an examining body, a court. The affidavit reference at the end is what the booking desk will ask for, and many papers print the affidavit number alongside the date. Everything else has to agree letter for letter with the affidavit and with the Gazette proforma, including the spacing in your surname. The four newspaper advertisement formats
Newspapers do not sell one thing called an advertisement. They sell four, priced completely differently, and knowing which you want stops the desk from upselling you.

One consequence is worth knowing before you argue with anybody. With a classified text advertisement you cannot buy a particular page or position, however much extra you offer. It goes where the paper puts change of name notices. Only display advertising lets you choose placement, and for a Gazette file placement makes no difference at all. Pay for the text ad.
Papers also sell enhancements: a side border, a background tint, bold type. They cost more and add nothing your file needs.

The single applicant rule is the newspaper's own editorial policy, not a Gazette requirement, and it matters where your case covers a parent and a child together. Confirm the format with the paper rather than assuming either way; the Gazette side does accept a properly worded multi-person correction notice.
Why you keep the complete page and not the cutting
Every instinct says to cut the notice out neatly and file it. Do not. A cutting proves that a paragraph was printed somewhere, by somebody, at some time. A complete page proves which newspaper printed it and on what date, because the masthead and the date are at the top of the page and not next to your notice.

The third wording is the one that education and family cases usually need, because nothing has been changed and a record is simply wrong.
