Your Gazette file is judged on paper, not on explanation. If one item is missing, older than a year, or spelt differently from the others, the file comes back and the fee is gone. How you assemble the Gazette file in the right order
The documents depend on each other. Make them in this sequence and each one copies from the last.
Lock the spelling in writing
Decide the exact old name as it appears on your ID and the exact new name you want, including spaces, initials and dots. Write both down. Everything below copies from this line.
Affidavit
Typed from the locked spelling, notarised in your district. Date it, because the one-year clock starts here.
Two newspaper notices
Same wording as the affidavit, one Bengali daily and one English daily. Collect the complete original pages the day they print.
Proforma, undertaking, request letter
All three typed from the locked spelling. You and two witnesses sign the proforma, you sign the undertaking and the letter, all in your old name.
Soft copy and certificate
The notice typed as a digital file, witness portion removed, old name typed in the signature place. You sign the certificate that it matches the hard copy.
IDs, photographs, fee
Self-attest the ID and address proof copies and the two photographs, pay on BharatKosh, print the receipt, and the file is complete.

The name change affidavit in your Gazette file
Your affidavit is a sworn statement on non-judicial stamp paper. It states your old name, your new name, your address in Tripura, your father's or husband's name and the reason for the change, and you sign it before a notary with a witness. This is the one document in the file that must be made in person, in Tripura, because a notary has to see you sign.
What the Gazette office checks: that the old name matches your ID exactly, that the new name matches the newspaper and the proforma exactly, and that the affidavit is not older than one year. What we check before that: that a Bengali or Kokborok name has not slipped into a second spelling between your Aadhaar and the affidavit. You publish the same notice in two daily newspapers that circulate in Tripura: one Bengali, one English. The notice carries your old name, your new name, your address and the affidavit date. The Department of Publication wants the complete original page from each paper, with the masthead and the date visible, because that is how it verifies the notice was really printed. A cutting, a photocopy or a screenshot is not accepted.

These two pages are the only documents in the file you cannot make again cheaply. Once submitted they stay with the Department. Keep a clear scan of each before they leave your hands.
The proforma, the undertaking and the request letter
Three typed pages that most Tripura applicants have never seen until they need them.
Prescribed proforma
The Department's format for the printed notice: "I, [old name], son of / daughter of / wife of [name], residing at [address], have changed my name and shall hereafter be known as [new name]." Computer typed on plain paper, never handwritten. You sign in your old name, and two witnesses sign with their full names and addresses. Submitted in duplicate.
Undertaking
Your signed declaration that the old name, the new name, your father's or husband's name and your address as stated are true and correct to the best of your knowledge. For a minor, the guardian signs. For a Central Government employee, a deed of change of name under the MHA Office Memorandum of 12 March 1987 takes its place.
Request letter
A short covering letter addressed to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054, asking for publication of the enclosed notice in Part IV, listing the enclosures, and giving your phone number and email so the Department can reach you if there is a query.
The soft copy of the notice and the similarity certificate
The item almost every website gets wrong, and the one the Department describes in most detail.
Along with the printed proforma, the Department wants the same notice as a typed digital file, so that it can be set for printing without retyping. Three rules apply. The soft copy carries the notice text without the witness portion. In the place where your signature sits on the paper version, your old name is typed. And you sign a short CD certificate stating that the CD and the hard copy are the same, that you are responsible for any mismatch and for the quality of the CD, and that the CD is in MS Word format.
The soft copy goes on a CD in MS Word format, with a signed CD certificate, because that is what the Department's guidelines describe and what the Civil Lines counter accepts. A pen drive or an email attachment is not a substitute. The exact wording of the CD certificate, and of the proforma it must match, is on the proforma format page. Preparing the CD is one of the reasons the file goes through our Delhi office rather than by post: if the counter has any question about it, it is answered there and then, not sent back to Agartala three weeks later.
ID proof, address proof, photographs and witness details

The BharatKosh fee receipt for your Gazette name change
The Government fee is paid on bharatkosh.gov.in, the Non-Tax Receipt Portal, and the e-receipt printout goes in the file. The fee is commonly Rs 1,100 for an adult and Rs 1,700 for a minor, and around Rs 7,500 to Rs 9,000 for an applicant outside India. The Department revises these figures each financial year, so the exact amount is confirmed on the day you pay.

One year old, never returned, in your own name

Not older than one year
Every document in the file must be less than one year old on the day of submission. An affidavit from two years ago, or a newspaper notice from last year's failed attempt, cannot be reused. Make the affidavit, publish the notices and file within a few weeks, and this never bites.
Not returned
Documents once submitted are not returned by the Department in any circumstance. Your original newspaper pages and your affidavit go in and stay in. Scan everything before it leaves you, and keep a printed set. You will need the set again for Aadhaar, PAN and the passport office.
In your own name
The application is submitted personally or by post or courier, in the applicant's own name; the guidelines say no other channel, such as agents or advocates, is entertained. Document traps that catch applicants from Tripura
Two English spellings of one name
Debbarma and Deb Barma. Reang and Riang. Chakma and Chakmaa. Bhattacharjee and Bhattacharya. Your Aadhaar, your school certificate and your affidavit may each carry a different one. The file must carry exactly one. We check the old name against your ID and the new name against every page before anything prints.
Only one paper in your town
In smaller towns it is easy to find a Bengali daily and hard to find an English one. Circulation in Tripura is the test, not the town of printing. We book both papers from Agartala so the pair is always right.
Address proof from a village committee area
If your address proof is a certificate from a Village Committee or Gram Panchayat rather than Aadhaar, the address must still match the affidavit word for word. Aadhaar with the same address is the cleanest choice.
Posting originals to a fake address
Websites showing "Gazette office, Agartala, 110054" are showing a Delhi pin code with a Tripura label. Originals posted there are lost, and they cannot be replaced. The only office is Civil Lines, Delhi 110054.

- Tripura Gazette
- The weekly gazette published by the Tripura Government Press in Agartala. It carries Government orders and notifications and has no published route for a personal name change.
- Part IV
- The section of the Gazette of India that carries notices from private people rather than from the Government. A personal name change appears here and nowhere else.
- Proforma
- The typed page that becomes your Gazette notice. It is filed in two identical copies, signed in your existing old name, and witnessed by two people.
- CD certificate
- A signed page declaring that the soft copy and the hard copy of your notice carry the same matter, that you are responsible for any mismatch and for the quality of the CD, and that the file is in MS Word format.
- Request letter
- The covering letter addressed to the Controller, Publication Department, Civil Line, Delhi 110054, stating that the formalities are complete and the documents are enclosed.
- Affidavit
- A written statement you swear is true before a notary. For a name change it records your old name, your new name, your parent's or husband's name, your address and the reason.
- Deponent
- The person who swears an affidavit. For a minor's name change the deponent is a parent, never the child.
- Verification clause
- The closing paragraph of an affidavit stating the place and date and confirming that the contents are true to the best of your knowledge and belief.
- Two witnesses
- The Gazette notice carries two witnesses, each giving a signature, a full name and a complete address. Initials or a partial address are a common reason a file is returned.
- Newspaper notice
- The same wording published in one Bengali daily and one English daily circulating in Tripura. The complete original page of each is kept, because a cutting is refused.
- BharatKosh
- The Non-Tax Receipt Portal at bharatkosh.gov.in, the only accepted channel for the Central Gazette fee.
- e-Gazette
- The portal at egazette.gov.in where published notices are searched and downloaded free. Gazette publication has been digital only since October 2015.
