Notice that the child's old name is a single word because the birth certificate says so, the new surname is spelt as on the father's Aadhaar, and the child appears nowhere as a signatory.
The single-parent variant and the other parent's consent letter
Where one parent signs alone, the opening line becomes "I, [name], [father / mother] of the minor..." and point 5 says which of three things is true: the other parent has consented in writing, a court order allows the applicant to decide the child's name, or the other parent has died. The matching document is attached.

The guardian variant of the minor's affidavit
A court-appointed guardian, or an adoptive parent, is the deponent in place of the parents. The opening line reads "I, [name], the legal guardian of the minor [child's full old name], appointed by order dated [date] of the [court]", or "the adoptive [father / mother] of the minor under the adoption order dated [date]", and the order is attached as a certified copy. The rest of the affidavit is the same. For an adoption, the order does one thing this affidavit cannot: it also changes the parents' names on the birth certificate. Where the child's old name on the affidavit comes from
For an adult the old name comes from Aadhaar. For a child, the birth certificate is the root record and Aadhaar was copied from it, or from the school. Copy the old name from the birth certificate, letter for letter. If the school register spells it differently, the affidavit still uses the birth certificate spelling in point 3, and adds a line: "That the minor's name has also been recorded as [school spelling] in the admission register of [school], and both refer to the same child." The Gazette then prints the change from the birth certificate name, and the school updates its register to the new name on the Gazette, which closes the old mismatch at the same time.
