The proof for each part of the application
For the surname

You do not delete the old entry yourself and you do not need to visit your parents' constituency. The shift is what removes it.
The husband's name on the electoral roll
The roll carries a relation's name against each elector, and it can be the father's, the mother's or the husband's. After marriage a woman may replace her father's name with her husband's, and many do, but nothing requires it. Your right to vote, your EPIC and every use of it are unaffected by which relation is printed. Keeping your maiden name on the voter ID
No law in India requires a woman to change her name on marriage, and the electoral roll is no exception. If you keep your maiden name, file Form 8 for the shift of residence alone, so that you vote where you now live under the name you have always used. What matters is consistency: the roll should agree with your Aadhaar, PAN and passport, whichever name you keep. When the Gazette is needed for a voter ID change after marriage
The marriage certificate proves a surname taken from a husband. It does not prove a new given name, and it does not prove a surname that is not his. The BLO visit at the marital address
A shift of residence is verified where you now live, and the item that records the address is covered in the Form 8 guide. so the Booth Level Officer who comes is the one for your new booth, not your parents'. Two things make it easy: being at that address, or reachable on the mobile you gave, and having the marriage certificate and an address proof to hand. A member of the marital household can confirm that you live there. Why filing a fresh Form 6 after marriage is the mistake
Form 6 is the application for registration by a person who is not on any electoral roll. A married woman already registered at her parents' address is on a roll, so her application is Form 8, whether she has moved across the street or across the state. Filing Form 6 at the marital address creates a second entry for the same person. Being enrolled in more than one place is not permitted, the duplicate has to be deleted, and the process of sorting it takes far longer than the original correction would have. If it has already happened, tell the BLO at the new address and let the ERO delete the duplicate entry; do not simply ignore one of the cards.

The Department does not publish a request. It publishes a notice, and it wants that notice supported by a set of signed papers. Five items go in, and they are prepared in this order because each one takes its wording from the one before it.

- CD certificate
- A signed page declaring that the soft copy and the hard copy of your notice carry the same matter, that you are responsible for any mismatch and for the quality of the CD, and that the file is in MS Word format.
- Community surname spelling
- Bengali and Kokborok names commonly have more than one accepted English spelling, such as Debbarma and Deb Barma. Records that use different forms do not match for a verifying officer.
- Correction against change
- A correction brings a record into line with what was already written elsewhere. A change records a name nobody has recorded before. The two need different documents and different wording.

- Notary public
