Why a bank is easier than everything else on your list
A board holds the record of who sat an examination. A Registrar holds the record of a birth. Those are statutory registers, and an office holding one is deciding something about the world, which is why they are careful and slow and hedged with time limits.
A bank is not doing that. It holds a customer file so it knows who it is dealing with, under identification rules that apply to all banks. The request letter
Banks work from written instructions, so this letter is the application. It should be short and it should contain both versions of the name written out exactly, along with enough detail to identify the account without anybody looking it up twice.

Three details decide acceptance. A wrong version is quoted exactly as printed, spacing and punctuation included, and never tidied. Where several records carry it, each is named in turn with its own wrong version.
