The marriage carve-out in the Passport Rules
The amendment of 23 December 2016 to Schedule III of the Passport Rules, 1980 says that for married applicants applying for a change of name or addition of a spouse's name, for divorcees applying for a change of name or deletion of a spouse's name, and for re-married applicants, no proof of marriage or of its dissolution is required beyond the details provided in the application form. The same notification keeps the newspapers-or-Gazette and two-document requirement for changes of name in all other circumstances. In other words the rules deliberately separate marriage from every other reason for changing a name.
In practice the counter at PSK Agartala still asks to see something, and takes the marriage certificate, Annexure J, or the spouse's passport with your name endorsed. That is a verification habit, not a Gazette requirement, and it is satisfied by a document you already have or can sign in ten minutes.

Annexure J and when it saves a Tripura couple
Annexure J is a joint declaration by a husband and wife, carrying a joint photograph of the couple and signed by both, declaring the marriage and the applicant's changed name. It exists for the couple with no registered marriage certificate, which is still common in Tripura where the wedding is solemnised by community or religious rites and never registered. Why Aadhaar goes before the passport after marriage
The marriage carve-out removes the two-document rule, but it does not remove the counter's habit of reading your Aadhaar against your application. The new booklet and your old visas
A name change is a re-issue, so the passport you receive is a new booklet with a new number and fresh validity, and the old one comes back cancelled with your visas still inside it. A valid visa does not move to the new booklet. Many countries let you travel on the old visa carrying both passports; some require the visa to be re-stamped or transferred. Check the destination's rule before you book a ticket, and never dispose of the cancelled booklet while a visa in it is live. If a trip is close, consider whether the re-issue can wait until after it.
Police verification for a marriage name change
A re-issue on marriage, where the earlier passport was granted after verification and the address has not changed, is often granted without a fresh police check, or with a post-issue check. Where you have moved to the marital home, the new address usually triggers a verification at that address, so name the correct police station on the form and keep the folder ready for the officer. Divorce and remarriage on the same passport rule
The same provision covers a divorcee applying for a change of name or the deletion of a spouse's name, and a re-married applicant changing the name or the spouse's name. No proof of the dissolution is required beyond the form, though a counter commonly asks to see the decree. What a Gazette file contains, step by step
A complete file is five items long, and each one is checked at the counter. Preparing them in the right order saves a return journey, because a paper signed in the wrong name has to be redone from the beginning.

The signature on the proforma trips up more files than any other item. You are declaring that the person formerly called the old name has changed it, so the signature is made in that old name, and the new name appears only in the body of the notice.
