The two public documents in the new name, and which pair to use
The Passport Rules, as amended in 2016, ask a change-of-name applicant for the newspapers or the Gazette and for at least two public or school documents issued in the changed name, to show that the name has actually been adopted. In Tripura the fastest pair is Aadhaar and PAN, because Aadhaar is corrected on the Gazette in weeks and PAN then follows in ten minutes through Aadhaar e-KYC. Other documents accepted in practice are a voter ID, a driving licence, a bank passbook with photograph, a school or board certificate, or a Government service card, all in the new name.

Copies, self-attestation and what happens to your originals
Take one photocopy of every original on A4, single-sided, and self-attest each one: sign in ink across the lower part with your name. No Gazetted Officer's attestation and no notarisation is asked for. At counter A the originals are scanned and handed back; at counter B they are read against the application and handed back again. Nothing is retained except the old passport, which is cancelled and returned to you later with the new booklet. Carry the folder in one file, not loose, and keep a spare copy set at home in case a document has to be sent for police verification.

Does a POPSK ask for anything different
No, and a held file is handled the same way too; see if the passport office refuses. The only practical differences are the queue and the drive. What a Gazette file contains, step by step
A complete file is five items long, and each one is checked at the counter. Preparing them in the right order saves a return journey, because a paper signed in the wrong name has to be redone from the beginning.

The signature on the proforma trips up more files than any other item. You are declaring that the person formerly called the old name has changed it, so the signature is made in that old name, and the new name appears only in the body of the notice.

- The typed page that becomes your Gazette notice. It is filed in two identical copies, signed in your existing old name, and witnessed by two people.
- Affidavit
- A written statement you swear is true before a notary. For a name change it records your old name, your new name, your parent's or husband's name, your address and the reason.
- Re-issue
- A passport application that replaces an existing booklet. A change of name is a re-issue, so the number and validity are new and the old booklet is cancelled and returned.
- Annexure J
- A joint declaration by a husband and wife carrying a joint photograph and both signatures, used at a passport counter where there is no registered marriage certificate.
- POPSK
- A Post Office Passport Seva Kendra, running the same three counters and the same software as a Passport Seva Kendra, under the same Regional Passport Office.
- RPO Guwahati
- The Regional Passport Office holding jurisdiction for Tripura. It grants, prints and dispatches passports, and applicants never attend it in person.
- Newspaper notice
- The same wording published in one Bengali daily and one English daily circulating in Tripura. The complete original page of each is kept, because a cutting is refused.
- CD certificate
