Surname Change Affidavit in Tripura

Notice three things. The old name is a single word, because that is what Aadhaar says. The new surname is spelt "Debbarma" because that is how his father's Aadhaar spells it, and that spelling is now locked. And the surname clause names the surname once; the full names carry the rest.

The sentence that gets returned. "That I have changed my surname from Das to Roy." No full names, no new full name to print. The Gazette office needs the exact text of the notice, and the notice is "I, hitherto known as [full old name]... shall hereafter be known as [full new name]". Write the full names.

The community-spelling variant for Debbarma, Reang, Jamatia and Chakma

When the only thing changing is the English spelling of a Kokborok or Bengali surname, the affidavit does two jobs at once: it fixes one spelling for the future and it links the other spelling to you for the past. Add a fifth point:

5. That the surname Debbarma has also been written as Deb Barma in some of my documents, including my [Class 10 certificate / voter ID / PRTC], and that Rina Deb Barma and Rina Debbarma refer to one and the same person, that is, myself.

The surname change affidavit for a minor in Tripura

For a child under 18, the parents or guardian make the affidavit on the child's behalf. The opening line becomes "I, [parent's name], son of / daughter of [name], resident of [address], being the father / mother / guardian of the minor [child's full old name], born on [date]", point 2 reads "that I have changed the name of the minor from [full old name] to [full new name], by adding the surname [X]", and both parents sign as deponents where both are alive and available. Central Gazette
The Gazette of India, published by the Department of Publication at Civil Lines, Delhi. It is accepted in every office in India, including every office in Tripura, and by foreign missions.
Part IV
The section of the Gazette of India that carries notices from private people rather than from the Government. A personal name change appears here and nowhere else.
Community surname spelling
Bengali and Kokborok names commonly have more than one accepted English spelling, such as Debbarma and Deb Barma. Records that use different forms do not match for a verifying officer.
Proforma
The typed page that becomes your Gazette notice. It is filed in two identical copies, signed in your existing old name, and witnessed by two people.
Affidavit
A written statement you swear is true before a notary. For a name change it records your old name, your new name, your parent's or husband's name, your address and the reason.
Notary public
A person authorised by the Government to witness signatures and certify documents. Notarisation is done in person in Tripura and is the one step that cannot be completed online.
Newspaper notice
The same wording published in one Bengali daily and one English daily circulating in Tripura. The complete original page of each is kept, because a cutting is refused.
CD certificate
A signed page declaring that the soft copy and the hard copy of your notice carry the same matter, that you are responsible for any mismatch and for the quality of the CD, and that the file is in MS Word format.
Two witnesses
The Gazette notice carries two witnesses, each giving a signature, a full name and a complete address. Initials or a partial address are a common reason a file is returned.
Department of Publication
The office of the Government of India that publishes the Gazette, at Civil Lines behind the Delhi Vidhan Sabha Metro Station. It is the only office in the country that accepts a personal name change file.
Correction against change
A correction brings a record into line with what was already written elsewhere. A change records a name nobody has recorded before. The two need different documents and different wording.

The Non-Tax Receipt Portal at bharatkosh.gov.in, the only accepted channel for the Central Gazette fee.
