The surname change affidavit for a minor in Tripura
For a child under 18, the parents or guardian make the affidavit on the child's behalf. The opening line becomes "I, [parent's name], son of / daughter of [name], resident of [address], being the father / mother / guardian of the minor [child's full old name], born on [date]", point 2 reads "that I have changed the name of the minor from [full old name] to [full new name], by adding the surname [X]", and both parents sign as deponents where both are alive and available. - The Gazette of India, published by the Department of Publication at Civil Lines, Delhi. It is accepted in every office in India, including every office in Tripura, and by foreign missions.
