Reverting to a maiden surname, or dropping a married one, is a Gazette route with the decree of divorce attached as the reason document and an affidavit that says you are resuming your name before marriage. The one practical warning is the passport: a single-name passport is issued, but airlines and some countries handle single names badly, so people removing a surname often keep a second word, a father's name or a given middle name, in the surname box. Replacing a surname
Also a Gazette route. If the reason is adoption, the registered adoption order goes in the file and can also change the parent's name on the birth certificate; if the reason is choice, religion, community or family, your declaration is enough and the affidavit states it plainly. The passport: moving a word between boxes is not a surname change
Older passports from Tripura often carry the whole name in the given-name box with the surname box blank.
