Provident fund and insurance

Provident fund records are generally corrected through the employer rather than independently of it, because the employer certifies who you are. Trying to do it alone usually ends with being sent back.

The affidavit, locally

An affidavit is sworn rather than filled in, so you appear before a notary in person and sign in front of them. It supports the file but does not by itself satisfy an employer, which is why the departmental question comes first.

Where the Central Gazette fits, for an employee

There is no Gazette counter in Teliamura, in Khowai, in Agartala, or anywhere in Tripura. The file goes to one office in Delhi. For an employee the important point is that publication supports the departmental application rather than replacing it. The service book entry is still made by your own office, on its own terms. Anybody offering to file a Central Gazette notice within the state is describing something that does not exist.

What goes into the file

A Central Gazette file is five papers and the Department accepts or returns it as a whole. Nothing is returned afterwards, so it is assembled once and assembled properly.

Three details decide acceptance. A wrong version is quoted exactly as printed, spacing and punctuation included, and never tidied. Where several records carry it, each is named in turn with its own wrong version, which for an employee usually means naming the service record as well. And the proforma is computer typed in two copies, signed in the existing old name, with both witnesses giving a full address. Every variant is on the proforma format page.

The stages, and how it is filed from Teliamura

Only one office in India accepts a personal name change file, the Department of Publication at Civil Lines, behind the Delhi Vidhan Sabha Metro Station, Delhi 110054, and there is no counter in Agartala or Teliamura. Our Delhi head office lodges it in person. The local part stays in Khowai district: the affidavit sworn before a notary here, and both newspaper notices booked together so the file is not waiting on one of them. The Government fee is commonly Rs 1,100 for an adult and Rs 1,700 for a minor, paid on BharatKosh only, non-refundable and revised each financial year, with our service charge always given as a separate line before anything is paid. Publication appears in Part IV commonly 30 to 45 days after filing, on a weekly release cycle, and the copy is a free PDF from egazette.gov.in that never expires and needs no attestation for use in India. What about a nominee's name?+
That is a separate matter from your own name. A nominee name change is its own update, and insurance and provident fund nominations are handled separately from your service record, so do not assume one covers the other.
Is an affidavit enough for my employer?+
No employer of the kinds described here accepts an affidavit on its own. It supports the file, but the departmental process is what produces the change in your records, which is exactly why the question to your establishment section comes before any spending.
Is there a Gazette counter in Teliamura or Khowai?+
